Please Note:

For online registration – the registration confirmation receipts will be issued by Eventbrite on behalf of Canadian Society for Quality.

All fees noted are in Canadian dollar (CDN) with following points noted.

  • Tax – HST of 13% included in all registration fees; delegates pay credit card processing fee and admin charges.
  • Full 3-day conference includes: welcome reception, meals, refreshment breaks, banquet, admission to all Technical Sessions, exhibits and access to the online copy of proceedings.
  • One day (any day) or Workshop registration fees include: admission to technical sessions or workshop and meals on that day. Session attendees receive access to online copy of proceedings. Paper presenters can attend the workshop for free.
  • Group Registration Discount – for 3 and more people from the same organization, 4th registration is free. Each person must register separately and indicate other member’s names attending.
  • Spousal program – opportunities available, pre-register online.
  • Registration Cancellation Policy – Cancellation requests received in writing before June 1, 2012 will incur a $50 processing fee. There will be no refunds after June 1, 2012. Substitutions are allowed at any time.

 

Payment of Fees by Online
Online payment can be made via the secure registration form using Visa or MasterCard.

Payment by Cheque or Wire Transfer
If you would like to pay by cheque or by wire transfer (bank to bank) , please contact us by email: csq@shaw.ca to have confirmation and obtain the mailing address and / or the money transfer banking information. Please note to add the bank processing fee and our admin fee of CAD $10.50 that applies in all wire transfer payments.

Confirmation
Your registration and payment will be confirmed by receipts sent by email. Please use this confirmation as your registration receipt.

Student Registration
Please note that student and other special attendees who receive any discounts must provide their student or membership number or assigned code when registering to receive the discounted rate. Clerks at the registration desk will ask students and such affiliated members for proof at the time when they when pick up their name badges. If proof is not provided, the Clerks have a right to refuse the discount. A Full Conference rate will thereafter be applied.

Guest or Spouse Registration

  • Delegates are asked to register their guests or spouses for the Opening Reception, as numbers will be provided to the caterer in advance. Tickets will be given to the registered delegates for these guests to attend the function.
  • The quoted breakfast and lunch fees are on a per-person, per-day basis. Breakfast tickets can be purchased for June 27, 28 and 29. Lunch tickets can be purchased for June 27 and 28 only.
  • The Technical Sessions are not open for guests to attend. Should a guest wish to attend these sessions, they must register as a delegate and not as a guest.

Letter of Invitation to the Conference for Canadian Travel Visa
A letter of invitation is issued solely for the purpose of assisting participants with visa applications and/or to obtain funding for their attendance at the conference. Such a letter does not imply any financial obligation on the part of the conference organizers. If you require a letter of invitation, please contact the CQC Secretariat at csq@shaw.ca – However, you must first register and pay for the conference in order to receive the letter from the Secretariat. If your entry Visa to Canada is denied, we will refund your registration fee in full without any penalty.

Purchase of Congress CD Rom
The live Congress experience is a unique one, but not being able to attend does not mean that you need to miss out! Documentation of the event on CD Rom can be purchased for $400 CDN (includes shipping and handling). To order, send an email request to: csq@shaw.ca

Accessibility Requirements
Delegates are encouraged to inform the organizers should they have any accessibility requirements. There is a section on the online registration form where this information can be shared. Organizers will do their best to meet these requirements, and may contact registrants if need be for clarification or more information.
